II. Traffic Data Collected
When you visit our site these are the categories of information that are automatically tracked and collected:

- Type of Computer used to access the Site
- Type of Web Browser used to access the Site
- Referring source that may have sent you to the Site
- Other information associated with your browser’s interaction with the Site
III. Personal Information Collected
In order for you to be able to use certain Services or access certain areas of the Site, it's important for us to know who you are in order to address your needs. As a result, we may require you to provide us with certain information that personally identifies you otherwise known as “Personal Information”.
Personal Information can include the following,
- Contact Data (email address, phone number, MEDICWELL account password)
- Demographic Data (gender, date of birth, zip code)
- Medical Data (Previous doctors,dentists, other healthcare providers that you have visited, your reason for visiting the healthcare providers, your date of visitations, your medical history and other medical/health information that you share with us).
MEDICWELL may use small computer files called “Cookies'' which send information such as user ID, user preferences, lists of pages visited, and activities conducted on the Site to your computer’s hard drive. This helps us to improve our site through tracking how you use our website so that we may store your preferences. Cookies are generally used to customize your site experience and to store your password so that you do not have to enter it every time you visit our website. Cookies do not give us access to other information on your computer. You may block cookies or delete cookies at your own expense however, by disabling cookies you may not be able to access various features of the Site.
V. Web Beacons
In some cases, we may also use “Web Beacons” which are tiny graphic images files that are imbedded in a web page or email in order to collect anonymous non-personal information on how you use our Services, how you use sponsors/advertisers sites, and how you use the emails, special promotions, or newsletters that we have sent to you. Web Beacons also help us to statistically track how many people are using our Services, interacting with our emails, and our advertising sites.
VI. Website Analytics
We may also use third party “Website Analytics” that record things such as mouse clicks/movements, scrolling activity, and text typed into the Site. These Website Analytics do not track Personal Information, your account nor do they track browsing habits on other websites. The information collected is used to identify usability problems in order to make our Services easier for you to use.
All Traffic Data and postings are stored indefinitely even after “deletion” and this information may be stored indefinitely. Personal Information is stored indefinitely until you request us to either modify or delete the information.
VIII. Information Provided on Behalf of Children
MEDICWELL does not collect information nor does it provide any Services for people under the age of 18. If we discover that we have received information from a person under the age of 18, then we will respond directly to that child or their parent/legal guardian that a person under the age of 18 can not use our Services and then we will proceed to delete their information from our servers.
IX. HIPAA Compliance & Disclosure
- Comply with rules and regulations in regards to security and privacy of Protected Health Information (PHI) under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
- Require the use of a Business Associate Addendum (BAA) if such a case arises where confidential information is disclosed.
- Make certain that agents or subcontractors that work on our behalf also agree to these same restrictions.
- Protect against unauthorized use or disclosure of PHI by using no less than a reasonable amount of care.
- Not use/disclose any PHI unless we need to do so to meet contractual obligations or if we are required by law to do so.
- Report any non-compliance that we become aware of.
- Make PHI available at the request and direction of the customer if it is feasible to do so and in accordance with HIPAA requirements.
MEDICWELL will have named a HIPAA Security Official that creates, maintains, and trains regarding our HIPAA policies and procedures.
Upon the request of a termination of contract or the request of a customer, MEDICWELL, if feasible to do so, will destroy all PHI that was received or created as a result of any contract and retain no copies.
Upon reasonable notice and during normal business hours, MEDICWELL will allow the Secretary of the United States Department of Health and Human Services to audit our records and practices that relate to use/disclosure of PHI in order to ensure compliance.
X. Use and Sharing of Your Information
Based on your Personal Information we may recommend you certain resources or Providers. Based on your Contact Data we may contact you to remind you of upcoming/follow-up appointments. We may also utilize your medical data, Traffic Data, and demographic data in order to customize and tailor your experience of our Services whether through emails or other forms of communications as well as displaying relevant content that you may find yourself interested in. MEDICWELL may use your Personal Information to allow Providers to refer you to other Providers. We may also utilize anonymous statistical information to research trends on medical and health trends. The Personal Information used in such research would be conducted in an anonymous matter so that it can not be tied back to you. How we share different categories of information that we collect from you is outlined below:
You can choose to authorize MEDICWELL to share your Personal Information with certain Providers. For instance, if you complete a Medical History Form before an appointment, we may share your Medical History Form with the Provider if you agree that we may do so.
We may also share your medical data, contact data, demographic data, and Traffic Data with the Providers you have scheduled through our Services.
In order to customize your advertising experience, Personal Information may be shared with marketing affiliates and other third parties in an anonymized manner.
Anonymized Personal information that is shared with third parties is done to allow them to run statistical research on medical/health trends.
We may share contact data that you give us with subcontractors so that they may send you relevant promotional materials for MEDICWELL. Those subcontractors would be using contact data solely for that purpose.
We may also share anonymized Personal Information as well as non-personal information with business partners in order to determine how effective a given advertising campaign is.
XI. User Choice
You have the option to not provide us with any Personal Information but in such a case, you would not be able to access our services.

- Protect MEDICWELL's rights, property, safety and security and or that of another party.
- Enforce our Terms of Service
- Respond to claims that stipulate that any posting or other content violate the rights of third parties

Information that you choose to reveal in a review post or any other online discussion/forum is intentionally open to the public and thus is not private in any way. Think carefully before posting anything that discloses personally identifiable information. Third parties may be able to see and collect what you have written and use it in ways that we cannot control or predict.
XVI. Lost or Stolen Information
If your contact data is lost, stolen or used without your permission, you must notify us immediately. In such a case, we will proceed to remove that contact data from your account You must promptly notify us if your contact data is lost, stolen, or used without permission. In such an event, we will remove that contact data from your account and update our records accordingly.
XVIII. Controlling Your Personal Information
As a registered user of MEDICWELL’s service, you are able to modify some of the Personal Information that you have included in your profile.
If you request to delete your account, MEDICWELL will use commercially reasonable efforts in order to delete your account and its Personal Information however, it may be impossible to remove your account without MEDICWELL retaining some residual information. MEDICWELL does not have any obligation to delete any data or information that was provided on a Medical History Form after the termination of either your account or your Provider’s account.
